In order to perform allogeneic transplantation, the person from whom bone marrow will be taken must be fully compatible with the patient. This is very important in order to avoid any complications during the transplantation. Transplantation can be done from identical twins, from compatible family members, or from someone else who appears to be compatible in the results, although not from the family. However, recently it has been observed that bone marrow transplantation can be performed successfully from semi-compatible individuals. Let’s take a look at the details on this subject.
We consider the process of collecting and freezing one’s own stem cells and then re-transplanting them with high-dose chemotherapy as autologous, that is, self-stem cell transplantation. The aim here is to ensure that if there is a tumor hidden in the cells taken from the person, this tumor is destroyed by chemotherapy treatment. Afterward, stem cells are kept until the effect of chemotherapy wears off and they are successfully transplanted to the patient. In allogeneic stem cell transplantation, the cells are taken from someone else, not from the patient himself/herself. Of course, this person from whom bone marrow or peripheral blood cells will be taken must be compatible with the patient. After the tests, if they are compatible, a transplant can be made from the patient’s siblings, relatives, or from an individual who is compatible but not related.
Unfortunately, if the person’s family does not have a person compatible for the transplantation, a search for this transplant begins. From everyone whose tissue group antigens are compatible with the patient, 2 to 4 millimeters of blood is drawn and sent to the test. These tests, which come out in a few days, reveal whether the person can be a healthy donor. In this way, anyone who is healthy between the ages of 2 and 60 can have a bone transplant as a volunteer donor to those they are fully compatible with.
In the bone marrow transplantation unit, special systems are used to prevent people from getting infected and breathing easily. In addition to special ventilation systems, protection conditions are provided at the highest level in the units. There is a continuous cleaning with the HEPA filter while the air is working with positive pressure. The hospitalization period of the patient varies between 15 and 30 days, and the patient is not allowed to leave the unit without being discharged. Of course, how long he/she will stay depends entirely on his/her health.
